I give permission for my child noted on this enrolment form to attend and participate in The Yoga Hutt kids yoga classes and I consent to my child using the rest room (right near the room) without an adult but accompanied by a peer from the class.

I, as the guardian of the child on this enrolment form understand that while every care will be taken by the teacher as with all physical exercise, injuries are possible. I hereby agree not to hold The Yoga Hutt liable in the event of an injury. In the event of a medical emergency I consent to an ambulance being called.

I consent to Kristy Stuchbery touching my child named on this enrolment form for realignment and safety/instructional purposes and understand that even though this will be very minimal at times it may be needed in order for them to learn the correct pose or placement of their body.

I understand that the The Yoga Hutt fees are due by the invoice due date and that any missed classes are non refundable.
